Install Steps:
- Uninstall playstore version
- Install the modded apk
- Put obb data inside internal memory folder Android/obb/

Love this game. Special doctor who... but in the begin you need to start the specialty of the doctor. .. but when I press on it nothing happens... and you can't skip it.... so I'm sick at that point over and over

Try to use the playstore version to complete any tutorial
Often tutorials are scripted so if you have mods they will interfere with the tutorial

I can't see anyway around doing the tutorial at the start of the modded game.... i can't even turn on cloud saves to restore a previous game. please help.

As already stated in above post, install and played the original playstore game first and pass the tutorial with it
then sync save your account with facebook or ingame account (google account will not work) then uninstall playstore game
and install mod game and load you account which have passed the tutorial and then you can play the mod game

You cant login to google+, and i dont know how to save with facebook, theres no option, only Like and i dobt think this will restore a saved game to be used with the mod. So, how in the world we use this mod, we cannot use cloud storage eaither.

try this method
download original apk thru apkpure.com and use zipsigner on both the original apk and mod apk
then you can play the original apk till you pass tutorial and then install the mod apk over it to progess pass the tutorial without uninstalling and losing your save

ZipSigner - Android Apps on Google Play
Install & run zipsigner
choose in/output
browse to the folder where you placed the mod/original apk and select the mod/original apk
click on the Sign the File
and you're done

I think you got the instruction mixed up, follow below step and tell me which part is giving you the problem

- Uninstall everything.
- Zipsign the original game apk
- install and play through tutorial where you get stuck
- Zipsign the mod apk
- Install the mod apk over the original apk, you should now be after the tutorial
